Care Act Needs Assessment

To see if your relative is eligible for support from adult social care a Care Act Needs Assessment will need to be completed.

It is now possible to complete a form online which can be accessed through the Adult Social Care Information and Advice Hub on the county council website.  However, you can still request a face to face assessment if you would prefer.
